What is a Member ID?

The Member ID is the most important piece of information on the Financial Support Card. The Member ID is assigned to the patient during the prescription fill and allows them to receive financial assistance.

What is a Second Page Print-out?

Select programs use a second page print-out. This provides additional information that pharmacists can give their patients at the time of the prescription fill.

This may include:

  • Information about third-party programs.
  • Financial assistance information directed at the patient
  • Additional patient support information
